A registration document is provided to patients and caregivers in an effort to better serve, educate and prepare you for your visit. Our goal is for our patients to have a great experience with Kidney Associates. Completing each page of the new patient registration form as well as the financial agreement and privacy form gives our office the accurate information needed for good patient care as well as proper insurance billng.

Consent Forms

Financial Policy - When signed by the patient, this serves as confirmation that the patient has read and understood their financial requirements and responsibilities as well as the responsibilities of the insurance company and of Kidney Associates for services rendered.

Privacy Consent - Each patient consent section should be carefully read and signed to give authorization for medical services rendered and to comply with HIPAA regulations.
